Indications of Replet Plus

Acute Coronary Syndrome (ACS): It is suitable for reducing the incidence of myocardial infarction (MI) and stroke in patients with non-ST-segment elevation ACS [Unstable Angina (UA) / Non-elevation Myocardial Infarction (NSTEMI)] and acute ACS patients Elevation [ST infarction (STEMI)]. 

Recent MI, recent stroke, or established peripheral artery disease: For patients with established peripheral artery disease or a history of myocardial infarction (MI) or recent stroke, it is suitable for reducing the incidence of myocardial infarction and stroke.

Pharmacology

Clopidogrel is a prodrug. It inhibits platelet activation and aggregation by irreversibly binding its active metabolites to P2Y12 ADP receptors on platelets. A dose-dependent inhibition of platelet aggregation can be observed 2 hours after a single oral administration. Repeatedly taking 75 mg every day to inhibit ADP-induced platelet aggregation on the first day, and the inhibition reached a steady state between the 3rd and the 7th day. 
 

Aspirin inhibits platelet aggregation by irreversibly inhibiting platelet cyclooxygenase. Therefore, it inhibits platelet aggregation thromboxane A2, an effective platelet aggregation and vasoconstriction inducer.

 

Dosage & Administration of Replet Plus

The recommended dose of Clopidogrel is 75 mg once daily with or without food. No dosage adjustment is necessary for elderly patients or patients with renal patients. For patients with acute coronary syndrome (unstable angina/non-Q-wave MI), Clopidogrel should be initiated with a single 300 mg loading dose and then continued at 75 mg once daily. Aspirin (75 mg325 mg once daily) should be initiated and continued in combination with Clopidogrel. In studies it was found that most patients with acute coronary syndrome also received heparin acutely

 

Interactions of Replet Plus

Oral anticoagulants, NSAIDs, Analgin, SSRIs, CYP2C19 inhibitors can increase the risk of bleeding. Shows interactions with tonofovir, valproic acid, varicella vaccine, acetazolamide, and nicorandil.

 

Contraindications

This combination is contraindicated in the following situations: Allergic to API or any component of the product. Active pathological bleeding, such as peptic ulcer or intracranial hemorrhage.

 

Side Effects

The most common clinically important side effects are pruritus, Diarrhoea and rash. Infrequent events include intracranial haemorrhage (0.4%) and neutropaenia (0.04%).

 

Pregnancy

There are no adequate and well-controlled studies on pregnant women. It should be used in the first trimester and the second trimester of pregnancy only if it is clearly needed. It is contraindicated during the third trimester of pregnancy. It is not clear whether clopidogrel is excreted in human milk, but aspirin is known to be excreted in human milk. This drug should be discontinued during lactation.

 

Precautions

This combination can prolong the bleeding time. 
Thrombotic Thrombocytopenic Purpura (TTP): TTP is rarely reported after using this combination. 
Reye's Syndrome: Reye's syndrome can occur in people with symptoms of chickenpox, flu, or flu. Patients receiving this combination therapy or patients with a history of allergies to other thienopyridines have reported allergic reactions, including rash, angioedema, or haematological reactions.
