Indications of Liorel 10
Baclofen is indicated in-
- Multiple sclerosis flexor spasms with associated pain, clonus, and muscle stiffness cause spasticity.
- skeletal muscle spasms caused by rheumatic diseases
- Injuries to the spinal cord and illnesses of the spinal cord
- Cerebrovascular accidents, neoplasms, and degenerative diseases of the brain
Pharmacology of Liorel 10
Baclofen suppresses monosynaptic and polysynaptic reflexes in the spinal cord by activating GABAB receptors, which block glutamate and aspartate release. It may also cause CNS depression by acting at intraspinal locations. Baclofen has an antinociceptive effect as well.
Dosage & Administration
Adult & children over 10 years: 5 mg 3 times daily, preferably with or after food, gradually increased; max. 100 mg daily.
Children <10 years: Treatment is usually started with 2.5 mg (2.5 ml) given 4 times daily then raised according to requirement. Daily maintenance dose-
- 12 month-2 years: 10-20 mg (10-20 ml)
- 2 years-6 years: 20-30 mg (20-30 ml)
- 6 years-10 years: 30-60 mg (30-60 ml)
Interaction of Liorel 10
- If Baclofen is used with central nervous system depressants, alcohol, or synthetic opiates, more sedation may result. Respiratory depression is a possibility as well.
- Because combined therapy with Baclofen and antihypertensives is likely to cause a greater drop in blood pressure, the antihypertensive drug dose should be increased correspondingly.
- Baclofen and tricyclic antidepressants used together may increase the pharmacological effects of Baclofen, resulting in severe muscular hypotonia.
- There have been many instances of mental disorientation, hallucinations, headaches, nausea, and agitation in Parkinson's disease patients using Baclofen with levodopa and carbidopa.
- When MAO inhibitors and Baclofen are used together, the CNS depressive effects may be amplified. Caution is advised, and one or both medicines' dosages should be modified as needed.
- Because a synergistic interaction between Baclofen and magnesium sulphate or other neuromuscular blocking medications is theoretically possible, caution should be used while using them together.
Contraindications
Baclofen is not recommended for people who are hypersensitive to any of the product's ingredients.
Side Effects of Liorel 10
The most common adverse reactions associated with Baclofen are transient drowsiness, daytime sedation, dizziness, weakness and fatigue.
- Central Nervous System: Headache (<10%), insomnia (<10%), and rarely, euphoria, excitement, depression, confusion, hallucinations, paraesthesia, nightmares, muscle pain, tinnitus, slurred speech, co-ordination disorder, tremor, rigidity, dystonia, ataxia, blurred vision, nystagmus, strabismus, miosis, mydriasis, diplopia, dysarthria, epileptic seizures, respiratory depression.
- Cardiovascular: Hypotension (<10%), rare instances of dyspnoea, palpitation, chest pain, syncope.
- Gastrointestinal: Nausea (approximately 10%), constipation (<10%) and rarely, dry mouth, anorexia, taste disorder, abdominal pain, vomiting, diarrhoea and positive test for occult blood in stool.
- Genitourinary: Urinary frequency (<10%) and rarely, enuresis, urinary retention, dysuria, impotence, inability to ejaculate, nocturia, haematuria.
- Other: Instances of rash, pruritus, ankle oedema, excessive perspiration, weight gain, nasal congestion, visual disturbances, hepatic function disorders and paradoxical increase in spasticity. Muscular hypotonia of a degree sufficient to make walking or movement difficult may occur but is usually relieved by readjusting the dosage. For this purpose, the daytime dosage may be reduced and the evening dosage increased.
Pregnancy & Lactation
Baclofen is classified as a pregnancy category B3 because there is no evidence that it is safe to take while pregnant. Baclofen is able to pass through the placental barrier. Baclofen should only be given to pregnant women if the physician believes the potential benefits outweigh the risks. Baclofen is excreted in breast milk, but the amounts are so tiny that no adverse effects on the child are predicted, according to the data thus far.
Precautions & Warnings
- Patients with poor renal function or those undergoing chronic haemodialysis should get lower dosages (about 5 mg per day).
- Patients with spasticity, as well as psychotic illnesses, schizophrenia, depressive or manic disorders, or confusional states, should be treated with caution and constantly watched, since these diseases may worsen.
- Baclofen may be administered in individuals with epilepsy and muscular spasticity under appropriate monitoring and as long as effective anticonvulsive treatment is continued. After stopping Baclofen medication or overdosing, the convulsion threshold can be lowered, and seizures have been observed.
- In individuals who have or have had peptic ulcers, cerebrovascular disorders, or hepatic, renal, or respiratory failure, Baclofen should be taken with care.
- Breathing and cardiovascular function must be closely monitored, particularly in individuals with cardiopulmonary illness and respiratory muscle weakness.
- Neurogenic abnormalities influencing bladder emptying may improve with Baclofen therapy. Acute urinary retention can develop in people with preexisting sphincter hypertonia. In these situations, Baclofen should be taken with care.
