Pantoprazole is used to suppress gastric acid secretion where there is therapeutic benefit; namely, peptic ulcer disease gastroesophageal reflux disease non-steroidal anti-inflammatory drug (NSAID) ulcers Helicobacter pylori eradication (in combination with antibiotics)
Zollinger-Ellison syndrome
Pantobex 20 is a proton pump inhibitor that covalently binds to the H + / K + ATPase system on the surface of gastric parietal cells to inhibit the last step of gastric acid production. No matter the stimulation lasts for more than 24 hours, this effect will inhibit basic gastric acid secretion and stimulate gastric acid secretion.
Oral
Benign gastric ulcer:
Take 40 mg every morning for 4 weeks. If it does not heal completely, continue taking it for another 4 weeks.
Gastroesophageal reflux disease: 2040 mg every morning for 4 consecutive weeks, if not completely healed, continue for 4 more weeks; the maintenance dose is 20 mg per day, which can be increased to 40 mg per day.
Duodenal ulcer: Take 40 mg every morning for 2 weeks. If it does not heal completely, continue taking it for another 2 weeks.
Duodenal ulcers associated with Helicobacter pylori: Pantoprazole 40 mg twice a day is recommended, in combination with the antibacterial drugs
detailed below:
1 gram of amoxicillin and 500 mg of clarithromycin per day Twice for 1 week, or 250 mg of clarithromycin and 400 mg of metronidazole twice a week, twice a day.
Prevention of gastric or duodenal ulcers related to non-steroidal anti-inflammatory drugs: For patients who need to take long-term non-steroidal anti-inflammatory drugs, 20 mg per day.
Zollinger-Ellison syndrome: Initially 80 mg per day, adjusted according to response (up to 40 mg per day for the elderly); daily dose greater than 80 mg, divided into two doses.
Intravenous injection:
Duodenal ulcer and gastric ulcer: 40 mg once daily for 710 days
Gastroesophageal reflux disease associated with a history of erosive esophagitis: 40 mg once daily for 710 days
Prevention of rebleeding due to peptic ulcer: 80 mg IV, followed by infusion of 8 mg/hour,
for 72 hours.
Acid Inhalation Prevention: 80 mg IV every 12 hours for 24 hours, then 40 mg every 12 hours. Zollinger Ellison syndrome and other pathologies Long-term treatment of hypersecretory diseases: 80 mg intravenously every 12 hours, if necessary, increase to 80 mg every 8 hours, and higher doses can be titrated according to produce acid.
Pantobex 20 is contraindicated in patients who are known to be allergic to any ingredient in the preparation.
Pantobex 20 is well-tolerated in both short-term and long-term treatment. Headache and diarrhea are common side effects, rarely including abdominal pain, gas, rash, insomnia, and high blood sugar.
The FDA classifies Pantobex 20 as B pregnancy. However, there are no adequate and well-controlled studies for pregnant women. Because animal reproduction research does not always predict human response, the drug should be used during pregnancy only when clearly necessary. Pantobex 20 has been shown to be excreted in human milk. Therefore, one should consider the benefits of the drug to the mother and decide whether to stop breastfeeding or stop taking the drug.
Patients should be advised that Pantobex 20 mg tablets should not be broken, chewed, or crushed. Long-term treatment with Pantobex 20 may cause cyanocobalamin (vitamin B12) malabsorption or may increase the risk of osteoporosis-related diseases.
