Pharmacology
Cilnidipine is a novel 4th generation dihydropyridine calcium-channel blocker (CCB). It inhibits cellular influx of calcium by blocking both of L-type voltage-gated calcium channels in vascular smooth muscle and N-type calcium channels in sympathetic nerve terminals and causes vasodilation of peripheral resistance vessels and coronary arteries. It has greater selectivity for vascular smooth muscle.The blockade of N-type calcium channels affects predominantly peripheral nerve endings of sympathetic neurons, thereby dilating blood vessel by lowering plasma catecholamine levels. Thus simultaneous blockade of the L-type & N-type calcium channels result in significant reduction of the blood pressure without causing reflex tachycardia.
Dosage & Administration
Adults: 5-10 mg once daily after breakfast. Maximum dose: 20 mg once daily. The safety of Cilnidipine in pediatric patients has not been established.
Use in the elderly: Since the elderly may be more susceptible to hypotension, therapy should be initiated with the lowest possible dose (5 mg).
Interaction of Cilnivas 5
Cilnidipine can interact with quinidine, phenytoin, rifampicin, erythromycin, other antihypertensive drugs and antipsychotic drugs.
Contraindications
Cilnivas is contraindicated in patients with known hypersensitivity to Cilnidipine, cardiogenic shock, recent
myocardial infarction or acute unstable angina, severe aortic stenosis.
Side effects of Cilnivas 5
The most common side effects of Cilnidipine are: Dizziness; flushing; headache; hypotension; peripheral edema; palpitations; GI disturbances; increased micturition frequency; lethargy; eye pain; depression.
Pregnancy & Lactation
Cilnidipine should not be administered in pregnant woman or woman having possibilities of being pregnant. It is also advisable to avoid the administration of Cilnidipine to nursing mothers. However, if the administration is indispensable, the patient should be instructed to discontinue lactation.
Precautions & Warnings
Hypotension, poor cardiac reserve, heart failure. Sudden withdrawal may exacerbate angina. Discontinue in patients who experience ischemic pain following administration.
