Doxycycline hydrochloride is indicated for bacterial infections caused by the following sensitive microorganisms:
Doxycycline hydrochloride is a semi-synthetic tetracycline antibiotic with a broad spectrum of activity. Above all, it is a bacteriostatic antibiotic. It has a spectrum of activity similar to that of other tetracyclines but is particularly more active against Staphylococcus aureus and Nocardia. The drug is generally active against penicillin-resistant strains of Staphylococcus aureus and against strains that are already resistant to other tetracyclines. Some strains of E. coli, Proteus mirabilis, and Klebsiella, which are generally resistant to tetracycline, may be sensitive to doxycycline. In addition, 7090% of the various anaerobes sensitive to doxycycline and Bacteroides fragilis were more likely to be sensitive to doxycycline than other tetracyclines.
Doxycycline is active against most strains of Haemophilus influenzaea and is particularly useful against infections with H. ducreyi, Actinomyces, Brucella and Vibrio cholerae. It is also active against Nocardia, Chlamydia, Mycoplasma and a variety of Rickettsiae. Doxycycline is active against spirochetes such as Borellia recurrentis, Treponema pallidum and Treponema pertenue. It is also active against Plasmodium falciparum.
Usual dose: 200 mg on first day, then 100 mg daily for 7-10 days.
Severe infections (including refractory urinary tract infections): 200 mg daily for 10 days.
Acne: 100 mg daily.
Uncomplicated genital chlamydia, non-gonococcal urethritis: 100 mg twice daily for 7-21 days (14-21 days in pelvic inflammatory disease).
Capsules should be swallowed whole with plenty of fluid during meals while sitting or standing.
The absorption of tetracycline is impaired by preparations containing antacids containing aluminum, calcium or magnesium and iron. The absorption of tetracycline is also impaired by bismuth salicylate. Barbiturates, carbamazepine, and phenytoin reduce the half-life of doxycycline. Concomitant use of tetracyclines may make oral contraceptives less effective. Patients on anticoagulant therapy may require a reduction in their dose of anticoagulants. The use of tetracyclines in combination with penicillins should be avoided.
Patients who are allergic to any tetracycline drugs should not use doxycycline. Doxycycline is contraindicated in children under 8 years of age. Pregnant and breastfeeding women are also contraindicated.
Nausea, vomiting, diarrhoea, skin rashes, hemolytic anaemia, eosinophilia may be reported.
Doxycycline should be avoided during pregnancy, because of the risk of dulling and affecting the bone development of the fetus. Doxycyclines pass into breast milk and mothers taking these drugs should not breast-feed.
The use of drugs of the tetracycline group during oral development (second half of pregnancy, infancy and childhood up to 8 years) can cause permanent discoloration of teeth. Therefore, tetracycline should not be used in this age group.
