Indications

Lipired is prescribed for individuals with hyperlipidemias of type lla, llb, III, IV, and V who have not reacted sufficiently to diet and other suitable treatments.

 

Pharmacology

Fenofibrate is a derivative of fibric acid. After oral administration, fenofibrate is quickly hydrolyzed to its pharmacologically active form, fenofibric acid. Fenofibric acid lowers total cholesterol, LDL cholesterol, Apo-lipoprotein B, total triglycerides, and VLDL cholesterol. Furthermore, Fenofibrate therapy raises HDL and apo-proteins apoAI apoAII. Fenofibrate also lowers blood uric acid levels in hyperuricemic and non-hyperuricemic people by increasing uric acid excretion through the urine. The micronized version of Nofiate (Fenofibrate) absorbs better than the non-micronized formulation.

 

Dosage & Administration

• For the treatment of adult patients with hypercholesterolemia or mixed hyperlipidemia, the initial dose of Fenofibrate is 200 mg per day.
• For adult patients with hypertriglyceridemia, the initial dose is 67 to 200 mg per day.
• The dose should be individualized according to patient response. Fenofibrate should be given with meals thereby, optimizing the bioavailability of the medication. Or, as directed by the registered physician.

 

Interaction

Fenofibrate has been shown to enhance warfarin's anticoagulant effects. When used with an anti-diabetic medication, it may increase glucose tolerance and have an additive effect. Fenofibrate may potentially enhance cyclosporine's nephrotoxicity. Fenofibrate should be used with caution when combined with HMG-CoA reductase inhibitors due to an increased risk of rhabdomyolysis. Low-dose statins combined with Fenofibrate, on the other hand, appear to be well tolerated.

 

Contraindications

Lipired is contraindicated in individuals with Fenofibrate hypersensitivity, severe renal or hepatic impairment, existing gall bladder disease, breastfeeding moms, and photosensitivity to ketoprofen.

 

Side Effects

Gastrointestinal (e.g., nausea, anorexia, stomach discomfort), pruritus, urticaria, impotence, as well as headache, dizziness, vertigo, tiredness, and hair loss; myotoxicity.

 

Pregnancy & Lactation

Pregnant women should avoid using fenofibrate.

 

Precautions & Warnings

Patients with the renal disease require special attention because gradual increases in serum creatinine concentration or failure to follow dose limits might result in myotoxicity; stop if myotoxicity is suspected or creatinine kinase concentration rises considerably. For the first year, liver function tests are advised every three months.
