Asthma, COPD, and bronchospasm are all treated with doxophylline.
The drug doxophylline is a brand-new bronchodilator. The inclusion of a dioxolane group in position 7 distinguishes it from Theophylline structurally. Doxophylline relaxes bronchial smooth muscle by inhibiting phosphodiesterase 4 specifically. Doxophylline, unlike Theophylline, appears to have lower affinity for adenosine A1 and A2 receptors, which could account for the drug's improved safety profile. Doxophylline has been shown to suppress platelet activating factor (PAF) production as well as the creation of leukotrienes.
Elderly: 200 mg tablet two or three times daily.
Adults: 400 mg tablet two or three times daily or as prescribed by the physician.

Other xanthine derivatives should not be taken simultaneously with doxophylline. Xanthines have been shown to have toxic synergism with ephedrine. Concomitant therapy with troleandomycin, lincomycin, clindamycin, allopurinol, cimetidine, ranitidine, propranolol, and anti-flu vaccine, as with other xanthines, may reduce xanthine hepatic clearance, resulting in an increase in blood levels. There has been no indication of a link between Doxophylline serum concentrations and harmful occurrences.
In acute myocardial infarction, doxophylline is contraindicated. Individuals with hypotension, nursing women, and patients who have exhibited allergy to its components are also advised against using it.
Doxophylline seldom produces major adverse effects, however it can have the same consequences as taking too much coffee. Nausea, vomiting, headaches, upset stomach, and heartburn are some of the symptoms.
Animal reproduction studies show that doxophylline has no effect on reproduction capacity or causes fetal harm when given to pregnant animals. However, because there is minimal experience with xanthines during pregnancy in humans, they should only be given to pregnant women if absolutely necessary. Doxophylline should not be taken by breastfeeding mothers.
This IV infusion is associated with increased urine excretion of the trace elements copper and, in particular, zinc, which should be considered when dosing trace elements, especially during long-term IV nutrition. Premature newborns with serious illnesses have been found to have hyperphenylalaninemia. The phenylalanine level should be monitored in these patients, and the infusion rate should be modified as needed. In individuals with diabetes, severe heart failure, or renal function in combination with fluid limitations or oliguria/anuria of unknown cause, this should be used with caution. Exogenous insulin therapy may be required in patients with hyperglycemia.
